Requirements of parents/guardians: Willing an dable to participate in treatment planning.
Requirements of placing agency: Referring worker is expected to maintain contact wiht OLBR staff adn resident.
Mandatory admission requirements: Current psychological evaluation, physical exam within 90 days, birth certif., ss card, parent info. If temporary custody, copy of case treatment plan, I.L. Assessment (if 16), immunization records, custody order, medical card, current school records, IEP (if applicable).
Visitation policies: Parents are encouraged to visit - approved visitations.

A.       Counseling:  Group, individual and familhy counseling.

B.      Education:  REsidents will attend public school.

C.      Vocational:  Yes, through Merridian Technology Center.

D.      Medical:  Provided by local doctor.

E.       Recreational:  Residents are encouraged to participate in extra-curricular activities.  Residents enjoy the freedom of the acreage surrounding each house inlcuding fishing, camping, basketball and volleyball facilities.

G.     Treatment Plan:  Initial within 30 days, 90 day review, 6 months thereafter.
